At the Law Office of Laurie M. Thomas we require an appointment to meet with an attorney. We want ensure that we have enough time set aside to address all issues and concerns you may have.

The form below is a basic intake form to provide us with information that we will need to have when we meet with you for your appointment. Please fill in as completely as possible.

Please Note: Do not fill in this intake form unless you have a scheduled appointment with our office. Providing our office with potentially confidential information may affect your rights if we have a conflict. (i.e. we represent the other side) Scheduling an appointment allows us to do a conflict check before you provide us any confidential information.

Appointment Day Tips:

For Family Law (Divorce, Child Support, Child Custody, Modifications)

• Have a copy of all Orders, if applicable.
• Have a copy of any and all paperwork you have received from the other side.
• Take time before the appointment to write down any and all question you may have for the attorney and bring them with you. This allows both of us to effectively use our time together.
  
• Bring copies of 4 of your most recent paycheck stubs or other form of income verification.

For Personal Injury (Auto accident and workers' Compensation)

• Bring a copy of your police report from the accident.
• Bring a copy of your discharge paperwork from the hospital or clinic.
• Bring a copy of any bill received for medical treatment.
• Workers' Comp- Bring a copy of notice of claim.

Other Civil Action:

• Bring in copies of all paperwork received from the opposing party.
• Bring in any paperwork from the Courts.
• Write down any question you may have prior to our meeting.
